Fix cargo fmt and clippy warnings across the workspace e2f8d9133d

Cleans up pre-existing rustfmt drift and clippy warnings (redundant
field names, needless into_iter, if-let-None, missing Default, a
complex lexer return type, and two large enum variants) so CI can
enforce both checks from the start.

Runs cargo fmt --check, clippy -D warnings, build, and test on every
push to main and every pull request, against a rustup-installed
stable toolchain on the ubuntu-latest runner.
